Social Prescribing Programme for City & Hackney
Social prescribing is non-clinical support for patients with social, emotional or practical needs. Patients are linked with sources of support within the local community to improve their wellbeing, build their resilience and to promote social inclusion. Social Prescribing can be particularly appropriate for disadvantaged, isolated, vulnerable or marginalised groups whose needs may be best met from within the voluntary sector.
Patients who may benefit from a social prescription will be referred by a health professional to a link worker based near the Homerton A&E Department. The link workers have a background in counselling, social care or health and are fully aware of services and resources within the local community that can support the patients’ health and wellbeing or continued recovery.
Once referred to the service, an allocated link worker will contact the patient and carry out a holistic assessment within 4 working days.
