Public reps sought for transformation care workstreams

A partnership of health and social care organisations in the City of London and Hackney is seeking public representatives for four groups (called care workstreams) that will join-up health and social care services and improve health and wellbeing outcomes in the following areas:

  • Unplanned care
  • Planned care
  • Prevention
  • Early years and children

The public representatives will play an important role ensuring the work carried out in each care workstream includes service user perspectives. These roles will contribute to ensuring residents are at the centre of the local health and care system. Each workstream will look at services across City and Hackney, as well tailoring services specifically to City residents’ needs.
 
About the partnership
Hackney & City: Health and Social Care Transformation is a partnership to improve health and wellbeing outcomes for City of London and Hackney residents through closer working and integrating services between local health and care organisations. The partners include City and Hackney Clinical Commissioning Group (CCG), City of London Corporation and Healthwatch City of London.
 
About the roles
Applicants can be either City of London or Hackney residents. For more information about the roles and care workstreams please download the role description. Interested candidates should submit a completed application form, personal statement, CV (no more than 2 sides A4) and equality form to info@healthwatchhackney.co.uk for the attention of Jon Williams. The closing date is 5pm Friday 5 May. Appointments will be made following a short panel interview against the person specification in the week 8-12 May.
